A recovery that starts at home
Our home from hospital service is built around the recovery journey. Before discharge, our assessment team works out exactly what is needed and puts a plan in place, from transport to shopping, so the move home is smooth.
Once home, our carers help with daily living, guard against setbacks, and gently rebuild the confidence that an illness or injury can knock. We offer short term or longer term support depending on how recovery goes.
